Consent and Photo Release Waiver: In consideration of acceptance of this entry, I assume all responsibility for any injury, loss or damage which I/my child might suffer including any loss or damage to my/him/her property in connection with the Toronto Police Children’s Games. In addition, I, for myself, my child and my personal representatives, heirs or successors, hereby release, waive and forever discharge the City of Toronto, The Toronto Police Services Board, The Toronto Police Service, The Toronto Auxiliary Police Service, the Chief Of Police, and all their respective agents, officials, servants, contractors, representatives, elected and appointed officials, successors and assigns and Variety Village / Variety - The Children’s Charity of and from all claims, demands, damages, costs, expenses, actions, causes of action, whether in law of equity, in respect of death, injury, loss or damage, as I/my child may ever have in connection with the above event. I also grant the event sponsors and organizers the irrevocable right to use, for publicity, advertising or related purposes, my/my child’s name, voice or physical appearance or any comments of mine/my child’s concerning the event. Further, I consent to having my child photographed and/or videotaped while participating in the event. By signing this document, I acknowledge having read, understood and agreed to the terms set forth in the consent.*
