At Urban Health Partnerships (UHP), we invest in communities by co-designing sustainable solutions that promote long-term health and well-being. Through our innovative Community Liaison (CL) Framework, we’re not just addressing immediate needs—we’re building lasting foundations for healthier futures. By integrating local voices directly into our planning and execution phases, we ensure that every initiative is rooted in the real needs and aspirations of the communities we serve.
Through the CL Framework, we employ residents from the communities we help as pivotal team members. These Community Liaisons bring their personal experiences and insights, guiding UHP’ projects to more effectively address the unique challenges of their neighborhoods. From improving public health policies to enhancing local environments, our CLs ensure that change is not only achieved but is sustained long-term.
Our Community Liaisons have been instrumental in several transformative projects. For instance, through their efforts, we have successfully implemented nutrition campaigns in schools that aim to reduce childhood obesity rates in communities with fewer resources. These programs not only teach immediate healthy habits but also lay the groundwork for healthier future generations.
By sponsoring UHP, you’re investing in more than immediate changes—you’re helping us lay the groundwork for lasting health improvements. Whether you contribute on a one-time or monthly basis, your support enables us to expand the CL Framework, equipping more communities with the tools to shape their futures. Join us and be a part of this vital movement toward long-term well-being.
Thank you for your support.
