In the event of an emergency affecting the health or welfare of my child/student/participant, I give permission to the Youth Pastors, Youth Leaders, or Adult Chaperones to administer first aid and/or transport my child/student/participant to the nearest doctor or medical facility for further medical attention as deemed necessary. This includes but is not in limitation to emergency surgery or medical treatment. I also acknowledge that this individual action in response to the emergency will be held blameless and I assume all responsibility for any medical expenses that could occur due to my child's/student/participant(s) medical emergency; this would also include any traveling costs should my child/student/Participant need to return home early due to medical reasons or even disciplinary action.
