How many years have you been a part of Steamboat Dance Theatre*

We ask our members to volunteer for at least one SDT event outside concert season. Please indicate in which event you'd like to be involved. Please note that in order to keep membership dues as low as possible, we need member participation; we understand life is busy and if you'd like to make an additional donation in lieu of volunteering, you're welcome to do that! We also ask that you ATTEND SDT Events to show your support :)*

My participation may include rigging and use of lyra or aerial hammocks. I understand that participation in the Steamboat Dance Theatre Annual Concert, including rehearsals and performances, are or may be dangerous and do or may involve risks of injury, loss, or damage, including inherent risks, hazards, and dangers, whether foreseeable or unforeseeable. I acknowledge that these risks, hazards and dangers cannot be eliminated. I further acknowledge that such risks may include but are not limited to bodily injury, personal injury, sickness, disease, death, and property loss or damage. By participating in this activity, I am assuming the risks inherent in this activity and am releasing Steamboat Dance Theatre, its Board of Directors, Steamboat Springs High School, KPA Productions, Kelly Anzalone, Yoshi Yonekawa, sponsors, choreographers (“Relessees”) and any of their officers, agents, employees, successors/heirs/assigns from any liability for claims or lawsuit by the undersigned participant arising out of this activity. Any questions that occurred to me have been answered to my satisfaction, and I am participating in his activity of my free will. Inconsideration of my being permitted to participate in this activity and knowing the dangers, hazards and risks of such activities, and in consideration of being permitted to participate in this activity, on behalf of myself, my family, heirs, and personal representative(s), I, the undersigned, agree to assume all the risks and responsibilities surrounding my participation in the activity, the transportation, and in any independent activities undertaken as an adjunct thereto, and in advance release, waive, forever discharge, and covenant not to sue Releasees, from and against any and all liability for any harm, injury, damage, claims, demands, actions, causes of action, costs and expenses of any nature that I may have or that I may have hereafter accrue to me, arising out of or related to any loss, damage, or injury, including but not limited to suffering and death, that may be sustained by me or by any property belonging to me, whether caused by the negligence or carelessness of the Releasees, or otherwise, while in, on, upon, or in transit to or from the premises where the activity, or any adjunct to the activity, occurs or is being conducted. I understand and agree that the Releasees are granted permission to authorize emergency medical treatment, if necessary, and that such action by Releasees shall be subject to the terms of this agreement. I understand and agree that the Releasees assume no responsibility for any injury or damage that might arise out of or in connection with such authorized emergency medical treatment. It is my express intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my estate, family, heirs, administrators, personal representatives, or assigns, if I am deceased, and shall be deemed as a “Release, Waiver, Discharge, and Covenant” not to sue the above named Releasees. I further agree to save and hold harmless, indemnify, and defend Releasees from any claim by me or my family, arising out of my participation in such activity or activities. In signing this release, I acknowledge and represent that I have fully informed myself of the consent of the foregoing waiver of liability and hold harmless agreement by reading it before I sign it, and I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducement, apart from the foregoing written statement, have been made. I understand that I am not required to participate in this activity, but I want to do so, despite the possible dangers and risks and despite this release. I further state that I am at least eighteen (18) years of age and fully competent to sign this Agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my participation in this activity, and that I have adequate health insurance necessary to provide for and pay any medical costs that may attendant as a result of injury to me. I hereby agree that Releasees and Steamboat Dance Theatre may use my photographic image or likeness taken during my participation in any Steamboat Dance Theatre activity for any purpose including use in promotional materials and on the internet and waive any rights to pre-approval, royalties or other compensation arising from or related to the use of photographs or film footage, which shall remain the sole copyrighted property of the Releasees and/or Steamboat Dance Theatre. I further agree that this release shall be construed in accordance with the laws of the State of Colorado. If any term or provision of this release shall be held illegal, unenforceable, or in conflict with any law governing this release the validity of the remaining portions shall not be affected thereby. Steamboat Dance Theatre, its Board of Directors, Steamboat Springs High School, KPA Productions, Kelly Anzalone and Yoshi Yonekawa have the right to refuse entry into this activity if this form is not completed. If under eighteen (18), this form must be signed by a parent or legal guardian before you may participate in this activity. Please Complete, Sign and Date the Emergency Contact Information at the bottom of this Agreement. Individuals without a Completed Form Will Not Be Allowed to Participate in the Annual Concert. I have carefully read this AGREEMENT and I fully understand its contents. I am aware that this is a release of liability and a contract between myself and the Releasees. I am signing this document of my own free will. ON BEHALF OF ANY MINOR PARTICIPANT: In consideration of the minor participant named below ("Minor"), being permitted to participate in Steamboat Dance Theatre annual concert, I agree to the following waiver, release and indemnification. I certify that I am a parent/guardian with legal responsibility for the Minor, and I further do consent and agree to the Minor's release of each of the Releasees identified above from any and all liability, claims, or causes of action which are or may be incident to or arise from Minor's involvements in the above-described activities. I further agree that this Agreement includes any and all current and future claims or causes of action, foreseen and unforeseen, that may be raised by the Minor, myself, minor's heirs, my heirs, minor's assigns, my assigns, my next of kin and/or minor's next of kin. I have read and understand the terms and conditions of the Agreement and by my signature, I acknowledge that I approve of and agree to the same on behalf of the Minor. In addition, I agree to defend, indemnify and hold the Releasees harmless from and against any lost damages or fees, including but not limited to attorneys' fees, arising out of or related in any way to any suit, demand or claim brought by or on behalf of the Minor. I have read and understand the alcohol policy as below With the SDT using the Steamboat Springs High School (SSHS) facilities and complying with their policies, the SDT will not allow alcohol or tobacco smoking on the SSHS premises. Also due to the various age of participants associated with the SDT there will be a “no tolerance” policy of alcohol or intoxication allowed at any rehearsals, showings, or performances. With the above mentioned I, as a member of the SDT, am aware that with my commitment I am a representative of this organization and will conduct myself in a manner that promotes a positive attitude and influence to the community. I agree not to arrive to showings, rehearsals, or performances intoxicated, in possession of, or smelling of alcohol. If I fail to follow these policies as stated above, I will be asked to leave the premises with the strong possibility of being removed from my piece(s) or the concert, no exceptions. I am also aware the SSHS is a “Tobacco Free” campus and that it prohibits me from smoking on school property.*
