By year's end, we anticipate extending our reach to over 800 constituents, ensuring more people have access to essential healthcare. <a href="https://www.crescentfoundationscd.org/initiatives/american-rescue-plan-act-housing-program/" target="_blank">ARPA Program</a>This state and federally funded program has currently assisted 5 children under the age of 18 with lead remediation services, a crucial support costing $25,000 per family. <a href="https://www.crescentfoundationscd.org/initiatives/community-health-worker-program/#earth-program" target="_blank">E.A.R.T.H. Program</a>Since its launch in June 2024, the EARTH program has supported 39 constituents with comprehensive holistic therapies including mental health counseling, massage therapy, acupuncture, and for mothers-to-be, free midwifery and doula support. Participants who enroll in mental health counseling receive 10 free 60-minute sessions, with a value of $200 per session. The program also provides access to midwifery and doula services valued at $5,000 and up.  <a href="https://www.crescentfoundationscd.org/initiatives/bridging-the-gap/" target="_blank">Bridging the Gap</a>In the year following the program's launch in 2020, Crescent led efforts to improve the transition from pediatric to adult healthcare. Leadership and AdvocacyCrescent Leadership has made a significant impact by participating in over 20 national and international panels and conferences, and is actively involved in 3 adult hospital patient advisory councils within Penn Medicine.  Educational SupportDuring the CHOP SCD Walk this year, Crescent distributed 300 backpacks filled with school supplies to children impacted by Sickle Cell Disease (SCD) and their families. Each backpack provides essential tools for success in school.
